To fill out an application to become a THFHT patient please click here.  At this time we are prioritizing applications from people with proof of primary residence in the Municipality of Trent Hills with a valid Ontario Health Card who are not attached to a primary care provider (doctor or nurse practitioner) or who are attached to a primary care provider more than 50km away. 


Yes. If you are a patient of Doctors Ali, Beyls, Maraghi, Parks, Read, Sheps, Williams or Nurse Practitioners Dalby, Deviney, Lewis or Robichaud - you are a THFHT patient. Please visit out booking page.

THFHT formally recognizes the traditional keepers of this land and, specifically our neighbours of the Alderville First Nation, with a formal territorial acknowledgement. We reside on the Gunshot Treaty Lands of 1788.  On these lands and on the shores of the big lake, the Mississauga Anishinabeg 

(A-NISH-IN-NAW-BEK) met with the Crown to facilitate the opening of these lands for settlement. Let us be reminded of the responsibility we all have in making sure that we respect these lands and waters that give us life and sustain our livelihoods.
