how to book an appointment

PatientS of the trent hills family health team: open hours

During clinic hours, patients enrolled to a  THFHT physician or NP can book: 

  • with their provider (physician or NP)
  • for vaccination at their provider's clinic
  • with a program at their provider's clinic
  • for blood work at CMH Community lab 


Clinic Hours all clinics:

Monday to Thursday 8:30 am to 5:00 pm

Monday to Thursday 5:00 pm to 8:00 pm: booked on rotating basis.

Friday: 8:30 am to 1:00 pm

Accessible appointments

Mobility issues/use assistive devices: THFHT clinics are accessible facilities with wheelchair ramps, wide hallways, and elevators where needed. Wheelchair accessible washrooms with supportive railings are also available at all locations. 


Home visits: THFHT is actively working with regional partners in the Ontario Health Team-Northumberland service area to provide home visits to people with mobility, cognitive or other health issues that make it difficult to get to the clinic. Please ask your provider or a THFHT nurse if you would like to talk about home visits or send an email to admin2@thfht.com to request an appointment.   


Hard-of-hearing or hearing impaired:  If you prefer to book appointments via email send a message to admin2@thfht.com or use our online booking system. 


Seniors or visually impaired: We are happy to provide any written material in large print or to have staff assist you with wrriten documents and forms.  Please contact us at 705-653-1801 or email admin2@thfht.com. 


Palliative care: Our team is committed to providing the right care, at the right time, in the right place. If medically appropriate, you can request palliative care in your own home with visiting nurses, physicians and other health care providers. Please ask your provider or a THFHT nurse if you would like to talk about palliative care or send an email to admin2@thfht.com to request an appointment.

  • Colborne Rural Outreach Clinic - Call 289-265-0505 to book an appointment with a Nurse Practitioner. Clinic is located at 34 Victoria Street Colborne, ON. No walk-ins. For more info click here. 
  • Haliburton, Kawartha Pine Ridge Public Health District Sexual Health Clinics - call 1-866-888-4577, ext. 1205 to book an appointment. No walk-ins. Clinics are health in Port Hope, Lindsay, Campbellford, Brighton and Haliburton.  Pap tests for unattached patients can be booked. 
  • Cancer Screening for people without a primary care provider - read about accessing screening tests for bowel, breast, lung, and cervical cancer if you are an unattached patient.

If you do not have a health care provider (physician or NP) and you would like to sign up for our wait list, please send us a message. An application form will be emailed to you. All applicants must have proof of a PRIMARY address in our service catchment area and an Ontario Health Card.
