Effective March 31, 2023, Cough, Cold and COVID Assessment Centres are closing. Members of the general public with mild symptoms of COVID-19 generally do not require PCR testing to confirm the diagnosis. Options now include the following.

If you do not have a health care provider (physician or nurse practitioner) or your health care provider is more than 50 km from your home, please fill out the application form below and send by mail or email to us.
All applicants must have proof of a PRIMARY address in our service catchment area and an Ontario Health Card. Priority is given to those who live in Trent Hills.
