Strategic Plan

THFHT is dedicated to improving the quality of its services and programs. We are accountable to the Ministry of Health and Ontario Health Team-Northumberland to deliver annual strategic plans that are linked to quality improvement priorities (QIP).

Rural Focus

We envision a future where Trent Hills is well known as a vibrant and healthy rural community. We are people taking care of people, neighbours taking care of neighbours.

Our history

THFHT has been funded by the Ministry of Health since 2005 in the first wave of Ontario family health teams. THFHT has partnered with Ontario Health Team-Northumberland since 2019 when Ontario Health teams were created by the Ministry of Health.

Our Quality Improvement Plan (QIP) for 2023/24 focuses on access to primary health care that is safe, effective, timely and delivered efficiently and appropriately in a rural community where 13,891 residents of all ages are spread across a large geographical area.

Our Community

“Rurality” remains the central determinant of health in Trent Hills. Residents are more likely to experience lower income or have a fixed income, underemployment, workplace and occupational risks, lower education and literacy, isolation and transportation deficits, and access to care issues. Residents of Trent Hills may also have a "ruralized" health culture or practices.

Care priorities include management of chronic disease, cancer care, palliative care, preventative care and vaccinations, mental health, and acute episodic care.

Gap analysis suggests that populations experiencing significant health inequity in our community are:

(a) residents who are not attached to a primary care provider,

(b) residents who are attached to a primary care provider at a distance,

(d) residents experiencing mental health issues.

Access to quality primary health care for Trent Hills residents is our top priority.

By developing an innovative model of primary care and reorganizing our health human resources in 2022/23, we have been able to add over 1700 residents to THFHT who were not attached to a primary care provider. In 2023/24 we will continue to enrol unattached patients and to strengthen partnerships with regional health sector service providers and community agencies to broaden health care access.

In 2023 we will add new programs for screening, prevention and disease management that support patients and enhance health literacy and self-care skills.

These programs are delivered by registered nurses and health promoters providing the highest level of quality health care. Our programs for 2023/24 are: Older Adult Primary Care, Women and Children’s Health, Heart and Lung Health, Joint and Back Health (includes Falls Reduction), Screening and Prevention (cancer and non-cancer), and Cancer Survivorship. Programs will be supported by our website with content on health topics and local health care services and resources.

In April 2023 we are launching a mental health system navigation service ("helpline") staffed by Mental Health Coordinators.

Available daily from Monday to Friday, callers can reach a coordinator and get timely assistance to locate the mental health services and care that they need. This service is open to all residents of Trent Hills. Callers do not need to be enrolled with THFHT.

We will measure patient satisfaction and engage patients in our service plan and will measure care safety and efficacy with an audit of the percentage of non-palliative patients who are newly dispensed an opioid medication.

We will continue to measure patient satisfaction and will expand our survey questions to assess service delivery changes due to COVID measures. In 2022/23 we surveyed the broader community and this provided valuable information regarding resident access to health care services.

In 2023/24 we will measure the percentage of non-palliative patients newly dispensed an opioid medication within a 6-month period prescribed by any provider in the health care system.

THFHT formally recognizes the traditional keepers of this land and, specifically our neighbours of the Alderville First Nation, with a formal territorial acknowledgement. We reside on the Gunshot Treaty Lands of 1788. On these lands and on the shores of the big lake, the Mississauga Anishinabeg

(A-NISH-IN-NAW-BEK) met with the Crown to facilitate the opening of these lands for settlement. Let us be reminded of the responsibility we all have in making sure that we respect these lands and waters that give us life and sustain our livelihoods.
