A mental health system navigation service for any Ontario resident. You do not have to be a THFHT patient to use this service.
ConnexOntario is a confidential phone, chat, texting and email service that helps people in Ontario who need support and information about mental health and addiction services available in their community. If you have concerns for yourself, or are worried about a friend or loved one, ConnexOntario is accessible 24 hours per day, 365 days per year. All service is free.

This program is available to all Ontario residents 18 years of age or older. You do not have to be a THFHT patient to use this service.
Depression and anxiety are the most common mental health issues experienced by people in Ontario.
Adults with these concerns can access free, publicly-funded cognitive-behavioural therapy and related services through the Ontario Structured Psychotherapy (OSP) program. Services include self-led resources guided by a coach or therapist, and individual or group cognitive-behavioural therapy.

Teenagers, youth and children need specialized mental health care provided by professionals experienced in services for this age group. Below is a list of organization in our area that provide these services.
Rebound offers a wide range of supportive and proactive services and programs for children, youth, and their families. Services are private and confidential and include individual counselling, group programs, tutoring programs and family wellness. Rebound are located at 700 Darcy St. N, Unit 20 in Cobourg. Children’s Mental Health Services is committed to strengthening and supporting the emotional well-being of children, youth and families in the least intrusive and most confidential manner. Services include counselling and therapy, brief and complex services, crisis support, youth legal support, rapid response service and groups. Kinark provides community-based child and youth mental health with a full range of assessment and treatment for children and youth 18 years of age and younger with complex needs. Northumberland County Branch services include counselling (brief and complex), coping skills and cognitive behavioural therapy (CBT), crisis support, specialized consultations, urgent response services, positive parenting support and case management. Youth Wellness Hubs Ontario is a network of 27hub networks (with 5 more coming soon) operating across Ontario. These spaces offer a range of Integrated Youth Services in one place. YWHO provides youth aged 12 to 25 with access to free services including mental health and substance use counselling, sexual health services, primary health care services, employment, education and training supports, and other community and social supports to improve overall wellness. Kids Help Phone is Canada’s only 24/7 e-mental health service offering free, multilingual and confidential support to help all young people Feel Out Loud. THFHT patients wishing to access the Mental Health Program can refer themselves to the mental health team or speak with a THFHT provider (physician or NP) or nurse. Please self-refer by leaving a voicemail message at 705-653-1801 extension 4918 or by sending an email to mentalhealth@thfht.com .
Before having an appointment with a THFHT Mental Health team member, we ask that you fill out the Intake Form (see below) to help us triage your service needs.

The THFHT Mental Health services are not an urgent care program. Anyone with concerns for their own or someone else’s safety should call 911. Anyone dealing with thoughts of suicide or worried for someone else should call 988.

The THFHT Mental Health Intake Form is for THFHT patients to download and complete before their appointment with a THFHT mental health team member. This form can be downloaded in PDF or Word format (see below). If you do not have a printer, please speak to a receptionist at the Campbellford clinic and they will give you a blank form. Trent Hills Family Health Team (THFHT) patients 16 years and older.

Information discussed in mental health appointments is private and confidential and is not shared, even if you are under age 18y. We have very strict privacy and security policies in place to protect your privacy.
If we are concerned that a person is at risk of harming themself or someone else, or if there is a concern about a child's welfare, we are obligated to notify appropriate authorities so that everything can be done to help ensure everyone’s safety.
