Patients of the Trent Hills Family Health Team can book appointments with the Screening & Prevention Program.
Book a screening appointment with a Registered Nurse who will coordinate age-appropriate cancer and non-cancer screening tests to THFHT patients aged 40 to 75 years.
RN’s will meet with patients (virtually and/or in-person) to arrange updates to patient screening tests. Screening may include cancer screening such as mammograms (breast cancer), Pap tests (cervical cancer), and Fecal Immunochemical Tests (FIT kit that screens for colorectal cancer) and non-cancer screening tests such as blood pressure checks, blood work for cholesterol and blood sugars, and bone mineral density tests for osteoporosis.
In addition to screening, program RN’s can coordinate follow-up visits for vaccination updates, advanced care planning, or other tests that might be appropriate for some patients on a case-by-case basis (e.g., breathing tests and CT scans for smokers or people with breathing conditions).

Book prevention coaching appointments with a Lifestyle Coach/Personal Trainer for individualized sessions on healthy living to THFHT adult patients over 18 years.
Program Co-Lead and Lifestyle Coach/Personal Trainer Shelley Aggett provides individualized coaching and support for making healthy lifestyle changes. Sessions include healthy eating, healthy movement, sleep, stress management, and social connection and community. At the initial session, you and Coach Shelley will pick which sessions are best for you!

No. THFHT patients can call the clinics themselves and book an appointment.
Warkworth patients can now book Screening visits with the Registered Nurse in the Warkworth Clinic. All other THFHT patients (Campbellford and Hastings) for Screening visits and ALL Prevention visits should call the Campbellford clinic to book. Your doctor or NP must be part of THFHT.
You can still book appointments with your doctor or nurse practitioner provider to request screening tests. It is your choice who you speak with.
Not with the screening program but you can book an appointment with your provider and ask to discuss screening tests that might be appropriate for you.
Yes. The Women and Children’s Health program will book women for a Pap test with an RN. The Cancer Survivorship program will book follow-up and repeat screening for cancer survivors. The Older Adult Primary Care Program monitors screening for people over the age of 75 years. And we will be introducing more new programs, such as Heart and Lung Health where THFHT patients will be able to book appointments with an RN.
