Registered Nurses specialized in well baby and child health care provide a physical assessment, vaccinations and health teaching to THFHT families. All care is scheduled in accordance with Canadian Pediatric Society guidelines and the Publicly Funded Immunization Schedule of Ontario. Registered Nurses specialized in cervical screening provide Pap tests to anyone aged 21 to 69 years with a cervix (women, transmasculine and non-binary people) who is or ever has been sexually active. All care is scheduled in accordance with Cancer Care Ontario Cervical Screening Guidelines. Speak to a program RN or your provider (MD, NP) to find out when you should have your next Pap test. Registered Nurses specialized in health care of patients with Diabetes Type 2 provide assessment, screening and blood work updates, health teaching and management of medications in consultation with primary providers (MD or NP). When appropriate the RN will connect you with external resources such as diabetes education, foot care, and eye examination. THFHT patients can visit our booking page for details on getting an appointment.
Registered Nurses currently offer an anticoagulation medication management program to all THFHT patients taking Coumadin (Warfarin). Also offered is help to quit smoking. Registered Nurses can provide counselling support, links to external resources, and free smoking cessation aids to assist you. If you would like to access these programs please ask your THFHT physician or nurse practitioner to refer you to the program nurse.
Coming Soon! An expanded nurse-led heart health program for THFHT patients who would like help managing their blood pressure, cholesterol, congestive heart failure (CHF), atrial fibrillation (AFIB), and other cardiac and lung diagnoses. Watch this page for updates.
Registered Nurses specialized in oncology care and cancer survivorship meet with THFHT patients to provide support, symptom management during treatments and navigation of appointments, diagnostics, and specialist consultations. Therapeutic conversations and counselling are offered as needed to patients and families experiencing cancer. Group classes that focus on strategies and techniques to support you to better manage your mental health and well-being. Groups are led by Registered Social Service Workers and are centred on a variety of topics including Anxiety Management, Mindfulness, Cognitive Restructuring (Think Well, Be Well) and more.
